On September 29, 2015 #GWMSwpg Grade 6 female students can, if the family chooses, to be vaccinated for the Human Papillomavirus (HPV). We ask all families who received vaccination forms at the beginning of the year to return them to their homeroom teacher. On the form parents can give consent for the Winnipeg Health Authority to vaccinate their child(ren) or not at school. It is very important to return all completed forms whether or not the family chooses to vaccinate the child. George Waters form return rate is 30 points below the city average. Please help improve this by returning forms to home room teachers. If the family chooses to opt out of the vaccinations entirely or just at school please check off 'No' in the consent part of the form. Then sign and return the form to school.
